We are happy to offer all of the necessary Cedarbrook camp forms online.

All forms are available in both Microsoft® Word and PDF formats. You can fill out the Word documents online, print and sign; or print either format, and then fill out and sign.

The Cedarbrook Clue contains all the pre-camp information you will need. It includes a packing list, driving directions, the mailing address, how to contact the registrar or camp director, emergency contact numbers and other helpful information.

All four forms are required for each staff member.
